Massachusetts Tax Withholding
To make a change in your State Tax Withholding, please use this form.
Please note that if you want your State withholding to be the same as
your Federal withholding, you only need to complete the W-4 form.
Direct Deposit
Use this form to make changes to your direct deposit. If you
want to have any portion of your pay directed to a savings account, you
must get a Specification Sheet or a letter from your bank indicating the
Routing and Transit number along with your account number.
